Vulvar Contact Dermatitis

Vulvar contact dermatitis is an inflammatory condition caused by irritants or allergens. Symptoms may include chronic itching or burning. Differential diagnosis includes but is not limited to vulvovaginitis such as candidiasis, lichen simplex chronicus, lichen sclerosus, and vaginal atrophy. Diagnosis is based on history and physical exam findings, which may include erythema, swelling, scaling, excoriations, fissures, and less commonly, erosions and ulcers. Workup may also include a vaginal fungal culture and a vulvar biopsy. Treatment involves removal of the irritants or allergens, vulvar care, and symptom management. Symptoms can often be managed with a topical corticosteroid ointment with oral antipruritic medication as needed.
